Care management for loved ones - There are no simple solutions when making care decisions involving the elderly, disabled or chronically ill, but we can help make it easier. Acting as client advocates, medical advisors or surrogate family members, our care managers work with clients to assess needs and recommend options for the care of loved ones. Our case managers include registered nurses, vocational counselors and masters-level social workers with extensive backgrounds that power an understanding of the intricacies of the psychosocial, environmental, financial, legal and other health-related needs each client faces.
